Register at a health centre
Choosing a health centre means that you make an active choice regarding the health centre you wish to belong to. This is known as listing. Being listed means that you pay a lower patient charge when you visit your health centre.
As the choice of health centre is individual, each family member must personally choose a health centre they wish to attend. In the case of children under the age of 16, it is the child’s parent or guardian who chooses.
If you list your child at a health centre, children under the age of six are listed automatically at the child welfare centre, which is part of the health centre.
Listing applies only to health centres and child welfare centres.
Via 1177 Care Guide e-services
The quickest way of choosing a health centre is to use 1177 Care Guide e-services. Click on Choose a health centre to log on and then follow the instructions to choose the health centre where you wish to be listed. Listing takes effect the day after you have made your choice.
Arrange for a form to be sent to your home
You can complete the form on the health centre website and within a few days you will receive a completed listing form at your registered address. You simply sign the form and post it back to the health centre. Listing takes effect the day after the health centre has registered your choice.
List at the health centre
We will be more than happy to help you at the health centre. Go to the health centre of your choice and state that you wish to be listed. Ideally, you should do this before you need medical care as the listing only takes effect the day after you have been listed.
